To any members interested in presenting at this conference, here is your opportunity to put forth an abstract.

Does your work advance the practice of patient- and family-centered care? Do you work in partnership with patients and families to improve and redesign health care? Are you partnering with individuals with underserved and vulnerable communities to reduce disparities in health and health care?

If your answer is yes, then I strongly encourage you to share your expertise and accomplishments at The 8th International Conference on Patient- and Family-Centered Care-Promoting Health Equity and Reducing Disparities, being held June 11-13, 2018 at the Baltimore Marriott Waterfront in Baltimore, MD.

The conference will showcase exemplary programs that are dedicated to collaboration among health care professionals, patients, and families. IPFCC invites abstracts from hospitals, primary care and specialty ambulatory settings, long-term care communities, community-based programs, public health and mental health agencies, patient- and family-led organizations, research institutes, and schools of medicine, nursing, and other health and helping professions.

Abstracts are invited for the following key topics:
