Health Quality Ontario is seeking volunteers to join advisory committees that will help support the development of two new quality standards.

These committees will help outline for clinicians and patients what quality care looks like for:

• CHRONIC PAIN

• TRANSITIONS IN CARE FROM HOSPITAL TO HOME

**If you or someone you know is interested,**we are now taking applications from health care professionals, administrators, researchers, patients, family members and informal caregivers from across Ontario.
