      Hello, I am looking for help gathering feedback on a national campaign that
      targets the public. The attached pdf provides better viewing of the Ad that is
      referenced in the survey. This is the request arising from the National
      Collaborating Center on infectious disease:


      “Would you please share our request for input on the
      Canadian Antibiotic Awareness Campaign with your contacts (i.e. members of the
      general public, patients, patient advocates, patient advisory networks)?
      We’d appreciate receiving responses by November 5th if
      possible. Please feel free to forward this
      email.

      We
      ask that people complete this short survey, after reviewing the sample of
      campaign material. The questionnaire takes approximately 15 minutes to
      complete.

      The
      sample campaign material (a public service advertisement, for a magazine)
      appears on the first page of the online survey, but some people may have
      difficulty seeing the text, so they may like to view the attached file
      instead, then proceed to the survey.

        I got through to the survey by cutting and pasting, but could not see the ad. Is it online somewhere? I'll give it a try again later.

          HI Annette, The pdf file did not upload. I will try another approach (I'm a newbie). The image was a repeat image of the image that started the survey, but it was not clear enough---therefore the additional attachment. Thank you for your efforts. I'm not understanding why you had to cut and paste and will try this link from the PAN site myself.

          Thank you for this feedback and to those who will continue to push past these glitches.

            I see that the survey link does not work even though it is accurate, and even if you try to cut and paste into your browser. And it will not allow me to create the hyperlink. I'm sorry I can't explain why that is.

            But there is another way! If you type the URL into your browser, it will take you there.
