The NPAO (Nurse Practitioners Association of Ontario) are asking for patient representatives who have an NP who is Most Responsible Provider and could speak to transitions in patient care from hospital to primary care to come to the stakeholder engagement day Jan 25 in downtown Toronto for the Ontario Primary Healthcare Nurse Practitioner Program (PHCNP).

About 25 stakeholders from across the continuum of care, as well as provincial organizations, and the nursing education leaders from the PHCNP program, will gather to have discussions about the evolving role of Nurse Practitioners, and implications for partnerships and education. This day will inform strategic directions for the Ontario PHCNP Program for 2019-24. Participants will be expected to participate in small and large group discussions for the day. It would be excellent to have a patient representative for this as well.

There is a small travel budget for those who could not attend otherwise.
