      Here is an opportunity from SE Health. I hope to participate as a "caregiver". Sounds like an amazing opportunity.

      SE Health and its partner, AMS Healthcare (a foundation) are undertaking an ambitious project to reimagine nurse-led community healthcare [more information below]. In order to help ensure spread and scale of the model after the 4 year demonstration phase, we are assembling at small but mighty Advisory Council to guide and support the work.

      We seek a "patient innovator" to be a member of the Council. The individual is ideally an older adult from Ontario with lived expertise in receiving home and community care services from multiple service providers for an extended period of time (ie more than 90 days). The individual must have a skill set that allows him/her to meaningfully contribute to the shaping of this effort from multiple lenses: how care will be experienced on the ground through, how it will be operationalized (ie tech and staffing enablers), through to key system-level changes to enable the new model (such as policy and funding changes).

      What is the SE Health-AMS Healthcare program [naming of our program is underway!]?
      SE Health and AMS Healthcare are joining forces on a bold, multi-year journey to test and position for scale a 21st century model of home and community care, inspired by the Dutch Buurtzorg neighbourhood care model (that is now spreading to 20 countries but has yet to be adapted in Canada).

      The main purpose of the Advisory Council is to provide feedback, guidance and make recommendations to the core team, executive sponsors and partners for this program. This will primarily be done via Council meetings held quarterly for 2 hours, either at the start of the work day, or the end of the work day. Meeting participation has two modalities: Either in person at AMS Healthcare (Toronto Yorkville), or via videoconference.

      So far the following dates have been held in the calendar for Advisory Council meetings in 2019:
      • April 4 - 4-6 pm
      • June 3 - 4-6 pm
      • September 9 - 4-6 pm
      • December 2 - 4-6 pm

      There may also be some follow-up questions/discussions by email or phone, where warranted. We anticipate we may draw upon our patient advisor to help access other people and families for various codesign sessions across the province, for example.

      All time contributed by our patient advisor will be renumerated at a rate of $20/hour (example - in person meetings or workshops, teleconferece time, email time, or time spent helping with specific requests or making connections). Any costs incurred for travel or parking are fully expensible to SE Health [or we can schedule Uber or Lyft rides for you where feasible].
