Health Quality Ontario - Patient Guides

Health Quality Ontario is looking for input from patients and their caregivers on their patient guides. Patient guides are designed for various conditions to help patients be actively involved in developing their care plans with their health care provider. They will be using this feedback to create future guides.

You do not need experience with the condition in the guide, as the survey asks about format, layout and features of the guides.
