I am working with the Canadian Home Care Association on a Health Canada funded project on operational processes for home-based palliative care. As part of this project, they are interested in interviewing patients and caregivers on their experiences and suggestions for improvement in processes. This is a national consultation. Hi Alies. This can be archived - they have completed the consultation phase and are now developing high impact practices which will be implemented by teams across the county to help improve operational practices for home-based palliative care
