Payal Agarwal at Women's College Hospital is heading up a project to monitor patients who are COVID positive at home using a range of strategies and she is looking for a patient partner to help with the ongoing design and evaluation.

Anyone would be great, preferably someone local since the initiative is based at Women's College Hospital and serves the downtown population.

COVID@Home aims to support patients with COVID-19 (swab + or presumed) in their homes by building on their relationship with their primary care provider. The initial program will focus on supporting PCPs and patients in the downtown Toronto region. The intent is to partner with others over time to explore how learnings can be scaled to other regions.

See [one pager](http://patientadvisors.groupsite.com/file_cabinet/files/987758/download/COVID@Home one-pager .docx?m=1587138925) and Payal will follow up with more details. Contact: payal.agarwal@gmail.com
