The Ottawa Hospital is looking for operating room team members (any profession, trainee or non-trainee) and surgical
patients to help us identify the most critical actions for effective teamwork by completing this
online survey.

This survey is the first of a two-round Delphi survey. It will take approximately 15 minutes to
complete each round and if you complete all rounds, you can choose to be entered into a draw
for a $100 (CAD) Amazon gift card.

A large proportion of surgical patient complications can be prevented with improved teamwork
in the operating room (OR). We would like to make good teamwork easy for healthcare
professionals by developing a comprehensive, standardized, structured and actionable set of
teamwork behaviours that is both feasible and acceptable to perioperative stakeholders.
