    Policy Integration for Primary Health Teams across Canada - Jun 20, 2022

      We are conducting a virtual national knowledge translation event to share the findings of the

      study and gather feedback from researchers, policymakers, decision makers, health providers,

      patients, family members, and caregivers. We are looking for participants who would be

      interested in taking part in this event. It would be great if you could share this with your network.

      In this study, we learned about provincial and regional level policies[1] that support

      integration[2] in health services through primary health care teams, where two or more providers

      work together in the community to care for the health and well-being of patients living with two or

      more chronic health conditions. We also wanted to understand if and how patients and caregivers

      were involved in creating and applying policies related to primary health care teams. This study

      took place in four provinces in Canada—British Columbia (BC), Alberta (AB), Ontario (ON), and

      Quebec (QC). The results will provide important information on primary health care teams,

      including engaging patients and caregivers, to improve integrated health services.

      You are invited to participate in two different discussion sessions: interactive webinar and online

      feedback/discussion. We will present our research results and you can provide comments and

      feedback on these results. Anyone who is involved in work in this area or has an interest in the

      same, including patients and caregivers are invited to take part. For the interactive webinar

      discussion, we will also work together to create recommendations on policy for primary health

      care teams. Following the interactive webinar, we may collect further data via an online feedback

      and discussion (e.g., UBC Qualtrics survey). Our goal is to share our findings, gather your

      feedback, and facilitate the transfer of knowledge into action to improve care provided by primary

      health care teams for patients with two or more chronic conditions.
