Carolyn Steele Gray, would like to invite anyone interested in partnering on the grant submission. This project would support the development of a digital bridge to help individuals experiencing multi-morbidity and complex care needs to transition from hospital back to their community. The big emphasis of this project is supporting person-centred transitions, enabled by technology.

This would involve helping them to design the care pathway and modify technologies to fit. They take their partnerships very seriously so this would be an important role on the team for them - individuals interested in technology may find it particularly appealing.
