Patient/Caregiver engagement in CIHR Catalyst Grant - primary care and geriatrics

I am a researcher a researcher at WCH (Womens College Hospital - Toronto) and over the last few years, I have been working on a series of projects that use digital technology to collect patient reported data in primary care and facilitate the evidence-based care through improved patient self-efficacy and shared decision making. We have currently built projects to address exercise, post-partum mental health and smoking and tobacco use. All project were designed and built with strong patient involvement using user-centered design methods. Evaluations of each are underway. We would like to extend these projects towards care for the elderly. There is the CIHR Catalyst Grant for Patient-Oriented Research (link) that would be a great start to helping us understand how we can best serve the needs of elderly patients. We are looking for patient and caregiver partners to help us with this work.
