    Patient/Caregiver engagement in CIHR Catalyst Grant - primary care and geriatrics

      I am a researcher a researcher at WCH (Womens College Hospital - Toronto) and over the last few years, I have been working on a series of projects that use digital technology to collect patient reported data in primary care and facilitate the evidence-based care through improved patient self-efficacy and shared decision making. We have currently built projects to address exercise, post-partum mental health and smoking and tobacco use. All project were designed and built with strong patient involvement using user-centered design methods. Evaluations of each are underway. You can read more about the work here (scroll to the bottom).

      We would like to extend these projects towards care for the elderly. There is the CIHR Catalyst Grant for Patient-Oriented Research (link) that would be a great start to helping us understand how we can best serve the needs of elderly patients. We are looking for patient and caregiver partners to help us with this work.

            Good Morning,

            I currently work in long term care, i also was a caregiver for my father while he underwent treatment for esophageal cancer. I am interested in participating in any work for quality improvements.
