At OTN, we are working on analyzing how digital solutions can support patients to self-manage their chronic disease. However, we were hoping to evaluate and leverage existing work that has been done in this space through patient engagement initiatives to really understand patient needs. We are looking for the appropriate person/organization to discuss patient engagement work related to chronic disease management.

Hi Alies

I used to manage a telehomecare ad rapid response nurses. I worked with OTN , and we set up both telehomecare. I was in the early stages of implementation and then operations. This is when I worked at CCAC / LHIN( I used to work closely with OTN). Telehomecare and Rapid Response aim for chronic diseases management of CHF and COPD.

I am a nurse so I have some background in chronic disease management.

What is the commitment, timeline and anything else? Can you tell me please before I make a decision

Liana, I don't know the timeline but would encourage you to as it of Kiran and then make your decision.
