      Are you a patient in Ontario, Canada without a family doctor on the Health Care Connect waiting list? Have you been assigned a family doctor through Health Care Connect? Researchers @WesternuFamMed are interested in talking to you. Please contact pccovid@uwo.ca if you are interested in sharing your experiences.

      They will then provide them with the formal invitation and the Letter of Information and Consent.
