      Thank you so much Kurtis! I think you articulated the issues beautifully.

      I've been fortunate to have taken several courses in patient engagement wherein I'm interacting with like-minded individuals. But when it comes to practice, as you say, organizational behaviour is ingrained.IMO, it stifles real innovation and the drivers who have the resources, funding and political clout, are often not wiling to move on from the status quo.

      If infra-structure for P/E were built within an organizational framework, systems could hopefully work together. Right now, I see silos springing up, dependent solely on the goodwill of interested parties, a slow and inefficient method to effect change. The time and resources required for minute increments of change feels like the dark ages.

      The IHPME (Institute for Health Policy, Management and Evaluation) at U of Toronto held a workshop that looking at environmentally sustainable healthcare and patient engagement (PE). There was a good showing from the patient partner side.

      It was an encouraging experience since those of the environmental side were mostly new to PE. So this was a great opportunity to expand the influence of patients and caregivers in another aspect of healthcare.

      Perhaps some of the other PAN members who attended have some observations.

      Alies and I , along with Brian Clark and Emily Nicholas Angl created tools for patient and researcher orientation (two separate decks of slides, and also an evaluation tool, funded by the Ontario SPOR Support Unit and using Patients Canada as the vehicle. They have since removed the work and it is housed now on the OSSU site

      For me the issue was the over prescribing of Ritalin related drugs (stimulants) for school age boys. It seemed that areas in the province, that do not have as many if any specialists, tended to be areas where there was a higher rate of prescribing. It appears to correlate with areas that had fewer specialists where there was a greater likelihood of "over prescribing".

      There is a movement in both the US and here in Ontario called "the conscious child" where there is a real effort on the part of educators and parents to not use these drugs as a quick fix to control student behaviour. These drugs are not without side effects. I am not an advocate for using behaviour modifying drugs before looking at the environment and the child's learning style/needs to build for student success.

      in addition there are more men using this type of drug in middle age as a means to focus more when making presentations???

      As a former educator this concerns me when it comes to children. There may be grandparents in our group who witness this happening for their grandchildren.

      Kim, I've read the article by Carolyn Canfield and plan to read the rest when I am done moving house. It is a useful and interesting edition and more importantly, it adds to the body of knowledge about patient engagement which can no longer be seen as a fad. Cheers! Alies

      Succession planning is something that always needs to be somewhere on the radar. I notice the Canadian Medical Association officially recognizes their Past President, the current President, and the President-elect.

      You don't want to lose wealth of knowledge a patient advisor gains during their tenure, so I agree, Annette, that it is an issue.

      Then again, with variations in funding, capacity and organizational culture, I am sure things are all over the map when it comes to giving new opportunities to a PFAC member once their mandate is up.
